Diagnostic & Treatment Consultation: Once a consulation is purchased ($50.00 fee), a medical questionaire form will be downloadable. You are able to write as much as you need. Dr. Schwartz will review the completed questionnaire and will return a complete (1-2 page) description of the possible problems and all the diagnostic techniques available to accurately access your problem. He will also include a full therapeutic course of treatments for each possible conclusion.
