<br> <br> <h1><p>male problems</p></h1><br> <a href="http://www.wix.com/UDIDYA/MEDICINE-CONSULT" target="_self"><br> <p><p>home</p></p></a><br> <p><p>There are some general rules that men show in regards to seeking medical help that are universal: Never go to a physician unless there is no other choice. As men age they excercise less and less, while the opposite is true for women. Men do not talk about their health problems especially with their wives. So as a physician when a man makes an appointment, I know that it is potentially serious. Not that any other patient is over-looked but in general as I said before , men wait way too long to be seen. When we talk about male problems, it is typically a discussion about the parts of the body that differ than females. </p></p><br> <p><p>Some of the most common complaints that a male may have are erection problems (inability to aquire, sustain), infertility, urination problems, testicular swelling or pain, penile lesions or discharge, and lack of sex drive. These problems are usually addressed first with their family physician and then referred out to a Urologist. Below is some basic physiology that is important to know before you set foot into a physicians office. </p><p></p><p> <U>Blood flow </U>to the male reproductive organs is extremely important for more than just the obvious reason. As in all tissue, it carries oxygen, nutrients, hormones and eliminates waste. If blood flow is compromised either systemically or locally, male problems will occur. If there is atherosclerosis (plaque in the arteries) , commonly found at the heart or carotid arteries, than rest assured it will be all over the body including the male organs. Some measurements that can be performed are plethysmography and doppler Ultrasound. </p><p></p><p><U>Growth hormone(GH) </U> is a rarely evaluated hormone that is extremely important for the male. After body growth ceases, it allows for increased muscle mass, sex drive, loss of fat, energy levels, increased skin turgor among other things. Sounds like an anti-aging hormone doesn't it? It is generally measured by IGF-1 levels. <U>Insulin-like growth factor(IGF-1)</U> is not GH but is directly proportional to GH. There are really only two ways to directly test GH. One is a simple blood draw but since GH is released only once per day and absorbed by the tissues within fifteen minutes of release. So the problem is when to measure . The second way would be to do a GH challenge test. The patient arrives at 8:00Am and gives a blood sample for a baseline GH level after resting supine for one half an hour. Then an IV is given of Arginine or Insulin over the next half hour. Blood level of GH are taken at half hour intervals for two hours. The amount of rise of GH is then charted out. IGF-1 is easier, cheaper and as accurate. An <U>Insulin-like growth factor binding protein(IGFBP-3) </U> level should also be established because it allows IGF-1 and Gh to circulate in the body longer. There is a correlation between increased prostate cancer and lowered IGFBP-3 levels.</p><p></p><p><U>Testosterone </U> levels both free and total, along with <U>Sex Hormone Bing Globulin(SHBG) </U> are extremely important. Total testesterone is the bound and unbound form and is the most common measurement taken. Unfortunately you also need the unbound(free) level and SHBG levels to make an accurate diagnosis of proper testosterone levels. The reasoning behind this staement is as follows. Free testosterone is the active form. If you have normal or high levels of testosterone and low levels of free the chances of getting male related cancer increase. But if you have a normal to normal-high level of free and normal to low-normal level of total then your chances of cancer decrease. Of course this is not the only predisposing factor to cancer but everything you do </p><p>to lower your chances are helpful. If the SHBG is high, it takes testosterone from the free form to the bound form because of its high affinity. So what we are aiming for is a normal total, higher free and normal SHBG levels.</p><p></p><p>I always try to keep my male patients at normal levels of GH and testosterone for their age until 35, and then keep them at 35 year old levels for the rest of their lives.</p><p></p><p>As far as preventative steps are concerned, both cardiovascular excercise and regular use of the male gland is important. If you eat animal tissue then it is extremely important to eat organic. Estrogen hormones are used to put on weight in meats and even though men have to have a low level of estrogen it is unhealthy to artificially raise those levels. Proper zinc levels are important for sperm production.</p><p></p><p>Lastly, a lot of problems with men have to do with their minds. Male stereotypes can be just as harmful to their bodies as female stereotypes can be to females. Do not fall prey to it. Men need love, romance, caring, and compliments just like anyone else. If this is not happening in the household, it can lead to male organ related problems.</p></p>
